Thank you for your interest in participating in this year's National Girls and Women in Sports Day Clinic on Saturday, February 14. This form is to register a child for the clinic. In order to have an accurate count of participants, please fill out the form for each child participating. One submission per child is required.

Massachusetts Women's Basketball takes on Miami (OH) on Saturday, February 14 at 1:00 p.m. In celebration of National Girls & Women in Sports Day, The clinic will be held after the game in the John Francis Kennedy Champions Center. The first 200 participants to register will receive a free t-shirt courtesey of UMassFive College Credit Union and the clinic will conclude with an autograph session.

This is a great opportunity for all kids 12 years of age and younger to get to learn from Division I student-athletes and coaches. It is also a great event to bring a group or team out to and have a fun day together!

Please download, complete, and print out this waiver prior to arriving to the game. You will present this waiver post-game at the check-in table.
