Thank you for your interest in participating in this year's Leaman Legacy Clinic on Saturday, January 10. This form is to register a child for the clinic. In order to have an accurate count of participants, please fill out the form for each child participating. One submission per child is required.

The Jack Leaman Legacy Clinic is a basketball clinic that will start approximately 30 minutes after the completion of the UMass Men's Basketball game against Ball State. Game time is set for 3:30 p.m. The clinic will be held after the game in the John Francis Kennedy Champions Center. The first 200 participants to register will receive a free t-shirt and the clinic will conclude with an autograph session!

This is a great opportunity for all kids, 12 years of age and younger, to get to learn from NCAA Division I student-athletes and coaches. It is also a great event to bring a group or team out to and have a fun day together!

Please download, complete, and print out this waiver prior to arriving to the game. You will present this waiver post-game at the check-in table.
